In consideration of being permitted to participate in swim lessons provided by Swim Lesson Suzi (Suzi Spice), I, the undersigned parent, guardian, or adult participant, hereby acknowledge and agree to the following:
Assumption of Risk. I understand that swimming and water activities involve inherent risks, including but not limited to drowning, injury, and other hazards. I voluntarily assume all risks associated with participation in swim lessons.
Release of Liability. I hereby release, waive, discharge, and covenant not to sue Swim Lesson Suzi, Suzi Spice, and any property owners where lessons are conducted, from any and all liability, claims, demands, actions, and causes of action arising out of or related to any loss, damage, or injury that may be sustained by the participant during swim lessons.
Medical Authorization. In the event of an emergency, I authorize Swim Lesson Suzi to seek medical treatment for the participant if I cannot be reached. I am responsible for all medical costs incurred.
Pool Rules. I agree that the participant will follow all pool rules and instructions given by Suzi. Parents and guardians are welcome to observe from poolside. Parents are not in the pool during lessons unless they are the swimmer enrolled in the lesson.
Lesson Packages. Lesson packages do not expire. Refunds are not provided for purchased packages, but lessons may be rescheduled with reasonable notice.
Governing Law. This agreement shall be governed by the laws of the State of Florida.
